Resources

An assisted living tour checklist you can actually use.

Take the same questions to each visit. Record what the provider confirms, what still needs an assessment, and who answered. Include the prospective resident whenever possible. This checklist helps organize a decision; it is not a clinical assessment or facility certification.

Before the visit

Write down the preferred location, budget range, important routines, and non-negotiable needs. Tell the provider about material assistance requirements through the appropriate private intake process. Ask whether any need should be assessed before scheduling a tour.

Compare after the visit

Use three columns: Confirmed, Still to verify, and Does not meet our needs. Keep the actual quote and assessment separate from a brochure or verbal generalization. Do not convert this checklist into an automatic safety score.
