What we can help with today

  • Identify realistic settings for the person's care needs and budget.
  • Verify current availability with providers that could fit.
  • Coordinate provider assessments — virtual when time is short.
  • Compare costs and what each monthly quote includes.
  • Organize next steps with the family and the hospital or rehab team.

What CareWise does not do

  • Make medical discharge decisions — those belong to the treating team.
  • Determine Medicare, Medicaid, ALTCS, VA, or insurance eligibility.
  • Give emergency medical advice. For a medical emergency, call 911.

Ready to talk it through? It's free to families.

Tell us what the search needs to solve

Share the intended area, the anticipated timing, the type of setting being considered, and who is authorized to coordinate the decision. Identify any care needs that affect the receiving provider's assessment through the established intake process.

Keep the responsibilities clear

CareWise researches potential options and helps compare information and coordinate placement-related communication. The treating team provides clinical instructions. The receiving provider decides whether it can admit the person and what assessment, documentation, services, and pricing apply.

Prepare for the handoff

Ask the clinical team about medication instructions, equipment, follow-up care, and who to contact with concerns. Confirm who is arranging transport and what the receiving provider needs before arrival. Written instructions and a named contact help keep the handoff organized.

Resolve budget and program assumptions

A rehabilitation or hospital coverage arrangement does not automatically pay for a long-term residential move. Medicare generally does not cover long-term custodial care. Ask the relevant insurer or agency to explain the actual coverage.

If the family needs a planned short-term stay rather than a discharge placement, see our respite care placement page.

For Arizona, review ALTCS and paying for care. Keep funding that is pending separate from the move-in payment plan.

Bring concerns to the clinical team

When the plan is unclear or you believe a proposed setting cannot meet the needs, raise that promptly with the responsible discharge team. This website does not decide discharge readiness or individual appeal rights.

Contact CareWise for a placement conversation. Professionals can use the referral pathway. This is not an emergency service.

Frequently asked questions

Does Medicare pay for assisted living after a hospital stay?
Medicare generally doesn't pay for room and board in assisted living. Ask the discharge planner and Medicare directly about what your coverage includes; CareWise doesn't determine coverage.
How do families pay for care long term?
Common sources include private funds, long-term care insurance, and benefits such as VA or ALTCS for those who qualify. We can share general information; eligibility is decided by each program.
How fast can a placement happen?
It depends on care needs, current availability, and the provider's assessment. We work quickly to find openings, but we can't guarantee admission or a move-in date.
What is a provider assessment?
Before admission, each provider reviews the person's care needs — often in person or virtually — to confirm it can support them and to set the monthly price.
What information should we gather?
The expected discharge date, the care team's contact, current care needs (mobility, transfers, medications, cognition), budget and payment sources, preferred areas, and who is part of the decision.
