Senior Care Placement Advisor (W-2) — Licensed Social Worker / Case Manager

If you're a hospital or SNF social worker, you know the drill. Forty-plus cases. A discharge window measured in hours. A family in shock in the hallway, and the best you can offer is a list of names and a "good luck."

We are an Arizona senior placement agency helping families find assisted living, memory care, and residential care homes when a parent or spouse can no longer safely live at home. We're hiring a licensed social worker or case manager to carry a real caseload, with real tools, and the time to get families to the right place the first time.
What this is
- A relationship-based advisor role built around assessment, not volume for its own sake
- Monday–Friday. No weekend rotation. No on-call discharge scramble.
- Field-based: you'll meet families at hospitals, SNFs, communities, and on tours across an assigned Arizona territory
- A mature caseload of about 40 active families, and roughly three completed move-ins per week once you're past a 90-day ramp
- Real pricing, availability, and facility data through Mildred, our placement platform — not a stale spreadsheet
- An ethical process: written fee disclosures to every family, and a standing rule that you recommend the best fit even when another option would pay CareWise more

What you'll do
- Complete care, budget, location, and preference assessments with families
- Match families to appropriate assisted living, memory care, and residential care options
- Coordinate with hospitals, rehabs, SNFs, hospice, home health, and care homes
- Support tours, move-in planning, and family follow-up
- Build and maintain referral relationships in your territory
- Document activity, recommendations, and outcomes in Mildred — carefully
- Represent CareWise with professionalism, warmth, and ethical clarity
Required
- Active Arizona license: LBSW, LMSW, LCSW, or RN
- Background in hospital social work, discharge planning, SNF social services, hospice, home health, or case management
- Comfortable being measured on outcomes and balancing compassion with urgency
- Valid driver's license and reliable vehicle
Compensation
- Base salary $70,000–$98,000, placed into one of three tiers (Developing, Advanced, Senior) based on experience, license level, and existing referral relationships
- Plus commission of 12.5–15% on net placement revenue from families you source
- At standard production, total first-year compensation for most advisors lands between roughly $106,000 and $141,000
- 401(k), health insurance, dental insurance, two weeks PTO
- Mileage reimbursed at the federal rate
Schedule
Monday–Friday, full-time, in person. This is a set schedule, not a flexible one — families and referral partners need to be able to count on you.
Territory
Assigned case by case, based on where you already have relationships and where we need coverage.
