Independent Senior Care Placement Advisor (1099)
CareWise Senior Placement · Arizona · Independent contractor, flexible schedule
For people who already know how to produce and are tired of handing most of it to someone else.
You might be a placement agent who's built a referral book and wants a better split. A home health or hospice marketer who's ready to go independent. Someone already placing families on your own who's done chasing invoices and building spreadsheets of facility pricing.
CareWise is an Arizona senior placement agency with an ethical, data-informed model: real assessments, written fee disclosures, and recommendations based on the family's needs — not on who pays us the most. We're opening a limited number of independent advisor agreements for self-starters who can produce with limited assistance and hold to our quality standards. Both halves matter.
How it works
You're paid a percentage of net placement revenue CareWise collects on families you place. No base salary. No benefits. You set your own schedule. You choose your structure at signing:
- Supported — 50%. CareWise prepares facility-option reports, schedules tours, and coordinates discharge. You bring in and manage the family relationship and your referral sources.
- Self-directed — 60%. You run the whole process yourself on CareWise's platform and brand.
- High-production self-directed — 75%. Available once you've reached $500,000 in trailing-12-month net collected revenue. Nobody starts here.
What that looks like. At three completed move-ins a week and an average of $2,000 net per move-in — a conservative figure that accounts for placements like many ALTCS cases that pay no fee — that's $288,000 in annual placement revenue. Supported pays $144,000. Self-directed pays $172,800. Figures are before your own taxes and expenses, and are an illustration, not an offer.
What CareWise provides
- The CareWise brand and facility relationships
- Mildred, our placement platform: real pricing, availability, and facility data, plus your documentation and case tracking
- Invoicing and collections — you place, we chase the fee
- A set of marketing materials each quarter
- Some overflow families from our W-2 advisors — limited, and not something to build a plan around
What you provide
- The referral relationships you develop and your own lead generation
- Mileage, phone, any licensing, and marketing beyond the quarterly kit; advisors at the 75% tier cover all of their own marketing
- A valid driver's license and reliable vehicle
- Your own taxes, insurance, and business expenses
The standards don't flex
Independence covers your schedule and your pipeline, not the process. Every CareWise advisor — W-2 or 1099 — gives families written fee disclosures, recommends the best fit even when another option would pay more, and documents the work in Mildred. The production benchmark is about three completed move-ins per week; agreements are reviewed quarterly.
Who this is for
- You've placed seniors before, or you've sold or marketed into the senior care continuum and know the players
- Referral partners already return your calls
- You're type A about your pipeline and meticulous about your documentation
- You want upside tied directly to what you produce
Who this is not for
- You need base pay or benefits to make it work
- You're uncomfortable being measured on outcomes
- You see fee disclosures and family-first recommendations as obstacles
- You'd rather be told what to do each day
No license required. Social work, nursing, or case management credentials are welcome but not necessary.
