Farrah McCubbin

Background

Farrah's career has been centered around helping people and families navigate some of the most difficult and overwhelming seasons of their lives. She began her journey as a Long-Term Care Case Manager where she spent eight years supporting individuals and their families and found the work incredibly rewarding. From there, Farrah developed a passion in helping people navigate through the Arizona Long Term Care System (ALTCS) and understand the resources available to them. Through her years of experience, she has developed a deep understanding of the challenges families face when a loved one needs additional care and support.

Why CareWise

"Having worked as a Long-Term Care Case Manager, I saw firsthand how difficult it can be for families to find the right care for their loved ones. I often saw people living in homes where they were unhappy or where the care no longer met their needs. That experience made me appreciate how valuable CareWise is in helping families navigate such a difficult and emotional decision. CareWise provides the guidance and support families need to find a care setting where their loved one can feel comfortable, supported, and truly cared for."

Role at CareWise

I coordinate and assist with the ALTCS applications for clients referred to CareWise and also help families choose ALTCS contracted homes. I help families understand the ALTCS process, determine what is needed to move forward, and guide them through the entire process. My role allows me to combine my years of experience as a Case Manager with my knowledge of ALTCS to help make an often complicated and overwhelming process easier.
